About LuminHRT

LuminHRT is a hormone optimization and regenerative medicine practice based in Los Gatos, California. We provide telehealth-based clinical services including bioidentical hormone replacement therapy (BHRT), peptide protocols, medical weight loss support, and related wellness programs for adults across eligible states.

All clinical services are delivered by licensed medical professionals. LuminHRT does not offer emergency medical services. If you are experiencing a medical emergency, please call 911 or visit the nearest emergency room immediately.

Eligibility

To use LuminHRT's services, you must:

• Be 18 years of age or older
• Be a legal resident of a US state in which LuminHRT is licensed to provide telehealth services
• Provide accurate, complete, and current information during registration and intake
• Not be using our services as a substitute for emergency medical care

LuminHRT reserves the right to decline or discontinue services to any individual at our clinical discretion, including in cases where treatment may not be appropriate or safe.

Medical Disclaimer & Not a Replacement for Emergency Care

LuminHRT services are designed to complement, not replace, your relationship with your primary care provider. We strongly recommend maintaining regular contact with all members of your healthcare team and disclosing all treatments, including those received through LuminHRT.

Individual results vary. LuminHRT does not guarantee specific outcomes from any treatment protocol.

Telehealth Services

All consultations and follow-up appointments are conducted via secure video. By using our telehealth platform, you acknowledge and consent to the following:

• Telehealth may involve the electronic transmission of your personal health information
• Despite reasonable security measures, no digital transmission is completely risk-free
• You have the right to withdraw consent to telehealth services at any time
• In-person care may be recommended or required in certain clinical circumstances
• You are responsible for ensuring a private, suitable environment during your appointments

Membership & Payments

LuminHRT offers membership-based programs, including our 12-month membership plan. The following terms apply to all memberships and payments:

• Membership fees are billed as outlined at the time of enrollment and are subject to change with advance notice
• All fees are non-refundable unless otherwise stated or required by applicable law
• Promotional discounts (e.g., 10% off for new clients) apply only to the initial membership term and are non-transferable
• Lab work, pharmacy costs, and supplements are billed separately and are not included in the membership fee unless explicitly stated
• You authorize LuminHRT to charge the payment method on file for recurring membership fees
• Failure to maintain a valid payment method may result in suspension or termination of services

To cancel your membership, you must provide written notice at least 30 days prior to your next billing cycle. Cancellations do not entitle you to a refund of fees already paid.

Prescriptions & Medications

All prescriptions issued through LuminHRT are determined at the sole clinical discretion of your treating provider. LuminHRT does not guarantee that any specific medication will be prescribed. The following conditions apply:

• Prescriptions are issued only after a clinical evaluation and review of appropriate lab work
• Medications are fulfilled through licensed compounding or retail pharmacies; LuminHRT is not responsible for pharmacy processing times or availability
• You are responsible for understanding how to use any prescribed medication and must inform your provider of any adverse effects
• Sharing or misusing prescription medications is strictly prohibited

Lab Work & Diagnostic Testing

Comprehensive laboratory testing is a required component of our clinical process. By using LuminHRT's services, you agree to:

• Complete required lab panels at a facility designated or approved by LuminHRT
• Authorize LuminHRT to receive and review your lab results
• Understand that lab costs are the responsibility of the patient and are not covered by the membership fee unless explicitly stated
• Follow clinician guidance regarding timing and frequency of follow-up testing

Privacy & Confidentiality

LuminHRT takes the privacy of your personal and medical information seriously. We comply with applicable federal and state privacy laws, including HIPAA. Your health information will not be shared with third parties without your explicit consent, except as required by law or for purposes directly related to your care.

Intellectual Property

All content on the LuminHRT website — including text, graphics, logos, images, protocols, and educational materials — is the exclusive property of LuminHRT and is protected by applicable copyright and trademark laws. You may not reproduce, distribute, modify, or use any content from this site without prior written consent from LuminHRT.

Limitation of Liability

To the fullest extent permitted by law, LuminHRT, its clinicians, staff, and affiliates shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of our services, website, or any treatment outcomes.

Our total liability in connection with any claim under these Terms shall not exceed the total amount you paid to LuminHRT in the three (3) months preceding the claim.

This limitation applies regardless of whether the claim is based on warranty, contract, tort, or any other legal theory, even if LuminHRT has been advised of the possibility of such damages.

Governing Law & Disputes

These Terms and Conditions are governed by the laws of the State of California, without regard to its conflict of law provisions. Any disputes arising under these Terms shall first be attempted to be resolved through good-faith negotiation. If unresolved, disputes shall be submitted to binding arbitration in Santa Clara County, California, in accordance with the rules of the American Arbitration Association.

You waive any right to participate in a class-action lawsuit or class-wide arbitration against LuminHRT.

Modifications to These Terms

LuminHRT reserves the right to update or modify these Terms and Conditions at any time. When changes are made, the "Last Updated" date at the top of this page will be revised. Your continued use of our services following any changes constitutes your acceptance of the updated Terms.

We encourage you to review these Terms periodically to stay informed of any updates.
